Evenflo Recalls Majestic™ High Chairs Due to Fall and Choking Hazards
 | Hazard: Plastic caps and metal screws on both sides of
the high chair can loosen and fall out, posing both fall and choking
hazards to children. Plastic caps and screws that become loose and fall
out can cause the seatback to suddenly fall back or detach from the high
chair. Children can fall out or collide with objects and suffer broken
bones, abrasions, cuts and bruises. Detached plastic caps and metal screws
also pose a choking hazard to children.
Incidents/Injuries: Evenflo has received 140 reports of seatbacks
reclining, falling back and/or detaching unexpectedly, which resulted in
at least 47 reports of bumps and bruises to the head, two reports of
broken bones, and at least 45 reports of other injuries including
abrasions, cuts, and bruises.
Evenflo has received more than 1,000 reports of plastic caps and screws
falling out of the high chairs, including seven incidents in which caps
and screws were found in children’s hands or mouths but were removed
before choking occurred.

News Release from CPSC: Defects Identified by CPSC Early Warning System
Prompt Crib Warning to Parents CPSC to consider rulemaking to address crib
defects
 | The U.S. Consumer Product Safety Commission (CPSC) is
urging parents and caregivers to closely inspect the hardware and
stability of their cribs to ensure all parts are in place and secure. The
warning by the CPSC comes after the agency’s Early Warning System has
identified concerns with the durability of cribs, especially those with
drop sides that can disengage and lead to dangers of entrapment and
strangulation. |

Infant Death Prompts Recall to Repair 985,000 Delta Enterprise Drop Side
Cribs; Missing Safety Pegs Can Cause Entrapment and Suffocation Hazards
 | The U.S. Consumer Product Safety Commission (CPSC), in
cooperation with Delta Enterprise Corp., of New York, New York., is
announcing the voluntary recall to replace missing safety pegs involving
985,000 drop side cribs. Failure to use or install safety pegs can cause
an entrapment and suffocation risk to infants and toddlers.
When the safety pegs in the base of each leg of the crib are missing from
the lower track, the crib locks can disengage and detach if lowered below
the peg hole, creating a hazardous gap. This gap can lead to the
entrapment and suffocation of an infant or toddler.
CPSC staff is aware of a death of an 8-month-old child who became
entrapped and suffocated when the drop side of the crib detached in a
reassembled crib where the safety pegs were not installed. CPSC is also
aware of two entrapments and nine disengagement incidents in cribs where
the safety pegs were missing.
The cribs were made in Taiwan and Indonesia |

Infant
Death Prompts Recall To Repair 600,000 Drop Side Cribs By Delta Enterprise;
Spring Peg Failure Can Cause Entrapment and Suffocation Hazards
 | The U.S. Consumer Product Safety Commission (CPSC), in
cooperation with Delta Enterprise Corp., of New York, N.Y., is announcing
the voluntary recall today of about 600,000 drop side cribs. The crib’s
drop side can detach when the spring peg is not engaged, which can cause
an entrapment and suffocation risk to infants and toddlers.
If the spring pegs in the base of each leg of the crib are pushed back
into the lower track, the pegs can become nonfunctional. This can cause
the drop side to detach from the crib and create a gap that can entrap an
infant or toddler.
CPSC staff is aware of a death of an 8-month-old child who suffocated when
the drop side of his crib detached. CPSC is also aware of one child
entrapment and one incident in which the drop side detached due to a
disengaged spring peg.
The cribs were made in China |

Infant Death Prompts Recall of Convertible Cribs by Playkids USA; Crib Poses
Entrapment and Suffocation Hazards
 | The U.S. Consumer Product Safety Commission (CPSC), in
cooperation with Playkids U.S.A. of Brooklyn, N.Y. is announcing today a
voluntary recall of about 2,000 convertible cribs. The sides of the
convertible crib are made of a mesh that expands, creating a gap between
the mattress and the side through which an infant can slip. This poses
suffocation and entrapment hazards for young children.
CPSC is aware of the death of a 5-month-old child on August 31, 2008 in
Brooklyn, N.Y. The child became entrapped between the mattress and the
stationary side rail of the convertible crib and suffocated. |

Six Retailers Agree To Stop Sale and Recall Simplicity Bassinets Due To
Strangulation Hazard
 | The U.S. Consumer Product Safety Commission (CPSC) is
urging retailers across the country to protect the health and safety of
babies by stopping sale and recalling nearly 900,000 dangerous Simplicity
bassinets. Yesterday, CPSC warned parents, caregivers and consumers to
immediately stop using convertible "close-sleeper/bedside sleeper"
bassinets manufactured by Simplicity Inc., of Reading, Pa., due to a
serious safety risk. Today, CPSC is announcing that the following
retailers have voluntarily agreed to recall the product and provide a
refund or store credit to consumers who return the product to the store
where purchased:
Additional retailers have subsequently agreed voluntarily to recall the
product and provide a refund or store credit to consumers who return the
product to the store where purchased are also listed. |

Infants Strangled to Death in Simplicity Bassinets: CPSC Urges Consumers To
Stop Using Simplicity 3-in-1 and 4-in-1 Convertible “Close-Sleeper” Models
 | The U.S. Consumer Product Safety Commission (CPSC) is
urging parents and caregivers to stop using convertible
“close-sleeper/bedside sleeper” bassinets manufactured by Simplicity Inc.,
of Reading, Pa. CPSC has learned that on August 21, 2008, a 5-month-old
girl from Shawnee, Kan. was strangled to death when she became entrapped
between the bassinet’s metal bars. This is the second strangulation death
CPSC has learned of in the close-sleeper bassinets. On September 29, 2007,
a 4-month-old girl from Noel, Mo. became entrapped in the metal bars of
the bassinet and died.
CPSC is issuing this safety alert because SFCA Inc., the company which
purchased all of Simplicity Inc.’s assets at public auction in April 2008,
has refused to cooperate with the government and recall the products. SFCA
maintains that it is not responsible for products previously manufactured
by Simplicity Inc. |
